  1. White surveys = males (100)

Beige surveys = females (100)

(It's really important to ensure the right colour is given to the appropriate gender as the question is not asked on the questionnaire - the analysis will be determined by the colour of the paper. Perhaps put a sign above each pile.)

  1. Provide a red or blue pen to respondents to ensure easiest data entry later. (Pencil or black pen are harder to see.)

  2. Keep a blank sheet of paper handy and jot down any relevant comments.

(eg., why someone refused to/couldn't participate and anything descriptive about that person eg., 20ish male said couldn't be bothered etc., any comments about the survey -eg that it was hard to understand etc, any comments like "I think Dr X is already doing all of this", "this is a great idea" etc - anything you think might be relevant.)

  1. Change the box on the counter every so often to ensure confidentiality between patients.
  1. Make a note of how many surveys remaining (for either males or females) when the first runs out. This is important also in the interpretation of the results.
